Job Description: Licensed Practical Nurse (LPN / T-LPN)Location: Concord, NH - Pleasant View Center Employment Type: Contract Assignment Dates: 12/01/25 - 03/02/26 Shift: 2nd Shift Hours: 3:00 PM - 11:00 PM Meal Break: 30 minutes Order Reason: New Opening Position Summary We are seeking a dedicated Licensed Practical Nurse (LPN / T-LPN) to join our team in Concord, NH for a 2nd shift contract assignment. The LPN will provide high-quality patient care, support daily clinical operations, and ensure a safe, compassionate environment. This role is ideal for a reliable, patient-focused nurse who excels in afternoon and evening care settings. Key Responsibilities Deliver direct patient care under the supervision of an RN or clinical leader Monitor patient conditions and report changes promptly Administer medications and treatments according to state guidelines Assist with ADLs and routine nursing tasks Perform wound care, injections, and clinical procedures as assigned Document care accurately and in compliance with regulations Maintain a clean, safe, and supportive care environment Communicate effectively with patients, families, and interdisciplinary teams Qualifications Active New Hampshire LPN license or compact equivalent Current BLS/CPR certification Strong assessment, communication, and organizational skills Experience in long-term care, skilled nursing, or rehabilitation preferred Reliable, compassionate, and comfortable working 2nd shift hours Shift & Assignment Details Shift: 2nd Start: 3:00 PM End: 11:00 PM Meal Break: 30 minutes Contract Dates: 12/01/25 - 03/02/26
