Responsibilities Home health setting providing skilled visits in patient homes Caseload includes cardiopulmonary, geriatrics, neuro, ortho, and oncology patients Typical schedule is day shift, Monday through Friday, with hours from 8:00 am to 5:00 pm Expected to complete a minimum of 6 to 8 home visits per day, subject to change at manager's discretion Weekend coverage required every third weekend Documentation completed using BayCare Forms, BayCare Scheduling, and Ace Timesheet systems Laptops, MiFi for internet access, and portable printers provided for field documentation Visits and hours are self-scheduled based on HomeCare office requirements Solid color scrubs required Main duties include patient assessment, individualized care planning, therapeutic interventions, and family education in the home environment Responsible for safety and injury prevention during all patient interactions Must provide documentation as per BayCare protocols using supplied technology Position is open to local candidates at the same rate Candidates may self-schedule visits and manage daily workflow independently Requirements Current state Physical Therapist license required if applicable BLS certification required Minimum of 2 years of recent physical therapy experience, specifically in home health Experience with assessment, cardiopulmonary and circulatory care, neuro and ortho specialties, and patient/family education Proficiency with equipment such as crutches, gait belts, hospital beds, walkers, slings, sliding boards, and oxygen delivery devices Familiarity with home exercise programs and recognizing signs of physical, sexual, or psychological abuse Valid driver's license and reliable car required for home visits COVID-19 vaccination or exemption form needed Flu shot required or declination with mandatory mask wearing while at work Benefits High Pay and Bonuses Medical, Dental and Vision Insurance with Day 1 Options 401(k) Plan Weekly Payroll Deposit Free Online CEUs Generous Housing Allowance Travel and Licensure Reimbursements Non-taxed Per Diem and Subsidy Traveler Rewards and Discounts American Traveler Job ID #P-673161.
