Job Summary The RN Case Manager will focus on developing and coordinating the multidisciplinary plan of care, emphasizing clinical care appropriateness, medical necessity, and resource management. You will coordinate activities that optimize patient throughput, quality outcomes, and appropriate resource utilization. Essential Responsibilities Conduct comprehensive assessments of the medical and psychosocial needs of patients. Develop and document individualized case management plans that address clinical, psychosocial, and discharge needs. Lead interdisciplinary team meetings to collaboratively plan and assess patient care interventions. Evaluate admissions for medical necessity using established criteria and escalate issues when necessary. Acts as a liaison with physicians, patients, families, and hospital staff to ensure effective communication and care coordination. Maintain compliance with regulatory standards and quality initiatives, making recommendations as necessary. Document care coordination activities to ensure clear communication with the healthcare team. Identify barriers to care and implement action plans to improve processes and trends. Support patients’ post-discharge needs and ensure they receive necessary services. Advocate for the healthcare needs of patients at risk for complications. Qualifications Registered Nurse with an Associate's Degree in Nursing or Diploma in Nursing is required; Bachelor's Degree preferred. Current Florida RN license or compact licensure is required. Three years of nursing experience in an acute care setting is mandatory; case management experience is preferred. Certification in Case Management, Nursing, or Utilization Review is a plus.
