Join Our Team as an IRIS Consultant! Are you dedicated to empowering individuals to live independently and take charge of their own care? Become a part of Lutheran Social Services of Wisconsin and Upper Michigan as an IRIS Consultant and assist participants in creating meaningful lives through the IRIS Program (Include, Respect, I Self-Direct). This position is a full-time role of 40 hours per week, working from 8 AM to 5 PM, and includes a hybrid work environment with travel required (30-50%) across the Dodge/Jefferson County area and into surrounding regions such as eastern Dane County and Columbia County. What You Will Do: Establish trusting relationships with participants to support their goals. Assist in the development of IRIS Support and Services Plans tailored to individual needs. Maintain monthly contact and conduct quarterly visits to ensure participant engagement. Help participants understand and uphold their IRIS eligibility requirements. Connect with local agencies and ADRCs to enhance service options for participants. Document all contacts and visits in required systems within 2 business days. Conduct annual Long-Term Care Functional Screens as needed. Work independently while collaborating within a supportive ICA team. Qualifications: Bachelor's degree in social work, psychology, human services, or a related field OR 4+ years of experience in social services working with seniors or individuals with disabilities OR Current IRIS Consultant in good standing with DHS exemption. Bilingual candidates in Spanish, Hmong, Arabic, Russian, or other languages are highly encouraged to apply! A strong commitment to enhancing participant autonomy and meaningful living experiences. Proficient with smartphones, tablets, and various computer systems. Knowledgeable about human behavior, social interaction, and community resources. A willingness to respectfully serve diverse populations. Requirements: A valid driver's license and reliable transportation.
