ResponsibilitiesEvaluate patients and create individualized plans of care Providing 1 to 1 patient care Deliver hands-on, patient centered treatment and education Monitor progress and adjust care plans to achieve meaningful outcomes QualificationsLicensed Physical Therapist (or ability to obtain licensure in the state of Wisconsin) Passion for evidence-based, patient centered care About Compensation: $75K -$100K per year + productivity bonuses, with additional compensation opportunities for certifications and location differentials.
