Create patient-centered transition plans by integrating clinical guidelines and metrics to promote quality care and efficiency. Educate the community about our services and represent the organization at public awareness events. Qualifications Essential requirements: Graduate of an accredited nursing, therapy, or social work program. Current licensure in the state of practice. A minimum of one year of experience in home care. Knowledge of federal, state, and local laws and regulatory guidelines pertaining to hospice operations. Basic technology skills, including the use of mobile devices. Preferred qualifications: Experience and knowledge in hospice care. Additional Requirements: Valid state driver’s license. Maintaining automobile liability insurance as mandated by law. Dependable transportation in good working condition. Ability to drive safely in various weather conditions.
