Summary/Objective: As a private duty nurse, your primary function will be to provide exceptional nursing care to pediatric patients in their homes, collaborating with an interdisciplinary team, their families, and the referring agency. This position requires an experienced nurse, proficient in pediatric home health care, who is willing to travel and is confident in initiating care in a patient's home under the direction of a registered nurse. Essential Functions: Assist in identifying patient/client needs in a physical, psychosocial, and environmental context. Participate in care planning and implementation in alignment with the Plan of Care (POC). Utilize infection control measures to protect both staff and patient/client. Ensure continuity of care through high-quality service delivery. Communicate effectively with patients/clients, families, team members, and healthcare professionals. Complete timely documentation of care. Monitor assigned cases for compliance with payor requirements. Demonstrate commitment to professional growth and competency. Promote agency philosophy and policies for quality care. Preferred/Required Education and Experience: Graduate from an accredited school of vocational nursing. Current Texas license as a Licensed Vocational Nurse or Registered Nurse, or a multi-state license from a National Nurse Licensure Compact (NLC) member state. Current driver's license. One year of experience as an RN or LVN in a clinical pediatric setting is strongly preferred. Home health experience is a plus. Additional Information: Flexibility in hours/shifts is needed based on client/patient needs and authorized hours. You must be adaptable to fill essential shifts as required. Working conditions may involve travel to multiple patient homes in various environments, requiring flexibility and resilience. You'll need to be prepared for exposure to unpleasant weather and manage challenging conditions. Physical demands include prolonged standing and walking, the ability to lift up to 50 pounds, and the ability to handle stressful situations while meeting patient needs.
