Personal Care Assistant: As a Personal Care Assistant, you will play a vital role in enhancing the quality of life for our patients. Join us to provide compassionate care and support to individuals as they navigate the challenges of daily living. Deliver direct patient care under the supervision of a registered nurse, following the personalized Care Plan. Assist patients in self-administering medications by ensuring proper dosage and observing intake. Accurately measure and report vital signs, identifying and communicating any abnormalities to supervisors. Support patients with personal hygiene tasks such as bathing, grooming, shaving, oral care, and maintaining skin health. Help with feeding and communicate basic nutrition principles, while observing and recording food and fluid intake. Assist with toileting needs, including bed pan usage and catheter management. Facilitate patient mobility, exercises, and transfers, adhering to the Care Plan and ensuring safety. Document care provided and changes in patient health, ensuring clear communication with the health care team. Maintain a clean and safe environment while practicing infection control principles. Willingly assist with light household duties such as laundry, meal preparation, and housekeeping when needed. Required Experience/Skills: High school diploma or equivalent. Completion of a Certified Nursing Assistant or Certified Home Health Aide program within the last 24 months. Must meet state certification requirements. A valid driver's license, auto insurance, and reliable transportation are mandatory. Must be in good standing on the HHA Registry (if applicable) and have completed HHA/CNA training to work for a Medicare-certified agency. At least one year of experience in a Home Health Aide or Certified Nursing Assistant role within the last 24 months in a hospital, nursing home, or home health/hospice agency.
