About the Role We are looking for experienced, organized Registered Nurses to join our dynamic virtual care team. This fully remote role will combine care management, care navigation, and Annual Wellness Visit (AWV) chart preparation, enabling providers to deliver comprehensive, compliant, and patient-centered care. The RN Care Manager will: Manage medically complex patients Develop and monitor care plans across multiple programs Complete AWV chart preparation Close care gaps and support preventive and chronic care initiatives This position demands strong clinical judgment, excellent documentation skills, and comfort with telehealth workflows, allowing you to work independently across various platforms. Schedule & Compensation We offer flexible scheduling options with up to 40 hours/week availability: Core coverage: Monday to Friday, 8:00 AM - 6:00 PM EST Weekend coverage: Required 2 weekend days/month or 1 Saturday/month Evening/Night shifts available Compensation is set at $35.00/hour for clinical services as a contract/1099 position. What You’ll Do Annual Wellness Visit (AWV) Chart Preparation Prepare comprehensive AWV charts to support provider efficiency and compliance. Conduct virtual visits via Zoom, reviewing medical, surgical, and family histories. Summarize medication lists, preventive screenings, cognitive assessments, and more. Identify overdue screenings and care gaps. Ensure documentation meets all required guidelines. Care Management & Clinical Oversight Provide telephonic care management for patients with complex behavioral health and socio-economic needs. Conduct nursing assessments and develop personalized care plans. Utilize motivational interviewing techniques to engage patients. Monitor patient progress and adjust care plans as necessary. Educate and advocate for patients in a support role. Care Navigation & Gap Closure Identify and facilitate closing care gaps (screenings, labs, medications). Coordinate referrals and follow-up actions to ensure service delivery. Oversee care across multiple providers for quality assurance. Documentation, Quality & Compliance Maintain accurate and timely patient records. Effectively utilize electronic health record systems and platforms. Participate in professional development and training opportunities. What We’re Looking For Required: Active, unrestricted Registered Nurse (RN) license in a compact state. 2-3 years of clinical nursing experience. Experience in telehealth or virtual care settings. Strong clinical assessment and coordination skills. Excellent communication and organizational capabilities. Preferred: Experience with Medicare Annual Wellness Visits (AWVs). Certification in case management or gerontological nursing. Technical Requirements: Must use a Windows or Apple computer (no Chromebooks). Wired headphones required. Minimum internet speed: 50 MBPS download and 20 MBPS upload.
