Advanced Practice Clinician - Nurse Practitioner / Physician AssistantLocation: Greensboro, NC (Davidson and Guilford Counties) Schedule: Monday - Friday, 8 AM - 5 PM Setting: Senior Living Facilities Travel: Yes, between 5 assigned facilities during shift About the Role The Advanced Practice Clinician (APC) supports the Senior Community Care (SCC) program-an integrated, longitudinal care model focused on high-risk members residing in facility settings. In this role, the APC plays a vital part in coordinating clinical care, improving patient outcomes, and collaborating with an interdisciplinary team. This position offers a strong work-life balance, with no on-call, no weekends, and no holiday requirements. Primary Responsibilities Conduct comprehensive assessments for complex, high-risk members Manage acute and chronic medical and behavioral health conditions Ensure complete and accurate documentation, including ICD-10 coding Collaborate with members of the interdisciplinary care team Lead advanced illness and advanced care planning conversations Educate and counsel patients and caregivers Travel between assigned facilities within Greensboro, NC during work hours Required Qualifications Active and unrestricted NP or PA license in state of residence, or ability to obtain by start date Education Requirements: NP: Graduate of an accredited MSN or DNP program PA: Graduate of an accredited PA degree program Active national certification: NP: AANP or ANCC certification (FNP, ANP, GNP, or AGACNP) PA: NCCPA certification Active DEA licensure/prescriptive authority Ability to meet job-related physical requirements (lifting 30-lb bag, stairs, kneeling for assessments) Reliable transportation for travel between facilities Ability to obtain a collaborative practice agreement (if applicable by state) Full COVID-19 vaccination in accordance with federal, state, and client requirements Must pass pre-employment drug screening (UnitedHealth Group is a drug-free workplace) Preferred Qualifications 1+ year of clinical experience (long-term care preferred) Experience serving patients with complex medical, behavioral, or social needs Strong understanding of geriatrics and chronic illness management Knowledge of advanced illness and end-of-life care discussions Proficiency with electronic medical records and medical documentation
