Benefits: 401(k) Competitive salary Flexible schedule Job Title: Registered Nurse - Home Care Position: Registered Nurse Summary: The Registered Nurse is responsible for planning, organizing, and directing home care services aimed at improving the quality of life for patients. Roles & Responsibilities: Perform an initial, comprehensive assessment of the patient to determine home care needs. Create and implement a personalized plan of care with measurable goals based on nursing diagnosis. Conduct ongoing reassessments to adjust the patient's care plan as their needs change. Utilize health assessment data to establish nursing diagnoses. Communicate any changes in patient condition to the clinical supervisor promptly. Provide comprehensive nursing care in accordance with established nursing processes. Document nursing interventions and diagnoses to meet patient needs effectively. Administer medications and treatments as prescribed by the physician, following the care plan. Supervise LVNs and CHHAs while adhering to agency policies and procedures. Educate patients and families on medication management and strategies to prevent disease exacerbation. Coordinate care with all health providers and the ordering physician. Complete and submit clinical notes accurately and in a timely manner. Report any significant changes in patient condition to the physician and nursing supervisor. Ensure that the patient has all medications and necessary DME for safe care. Engage in QAPI programs and initiatives for continuous improvement. Participate in professional development and required skills training. Attend monthly in-services. Adhere to all agency policies, including Infection Control and Emergency Management. Comply with HIPAA requirements to ensure patient privacy. Qualifications: Active Registered Nurse license in the state. Current CPR certification. Valid driver's license and auto insurance. Previous experience as a professional nurse, preferably in home health, hospice, or public health (at least one year). Bachelor's degree in nursing is preferred but not required. Strong interpersonal and communication skills. Proficient in computerized clinical documentation. Able to work autonomously with a solid understanding of safety protocols. Graduate of an accredited nursing program.
