Key responsibilities include: Conducting patient visits and coordinating care among various healthcare disciplines Serving as the primary contact for all care providers involved in a patient's care Consulting with physicians and office staff regarding patient care specifics Performing accurate OASIS collections and maintaining an updated medication profile Ensuring timely communication of lab values to physicians Participating in weekly case conferences and monthly case manager meetings Qualifications: To qualify for this role, candidates must have the following qualifications: Current Registered Nurse (RN) license in the state of employment At least one year of clinical nursing experience preferred Proficient knowledge and skills in modern nursing practices Valid state driver's license and automobile liability insurance Additional Information: Enhabit Home Health & Hospice is proud to be an equal opportunity employer.
