POSITION SUMMARY: This position as a Speech Language Pathologist is to screen, evaluate, and provide treatment, and participate in discharge planning of residents where services are being provided. Speech Language Pathologist should be able to provide comprehensive diagnostic, consultative and treatment for residents with hearing, language, speech, voice, cognitive and/or swallowing disorders. The therapist's goal should be to maximize the communicative and swallowing competence of each resident. Therapist must demonstrate and exercise the knowledge and skills necessary to provide quality care appropriate to the age of the residents served. Therapist shall be professional, ethical, efficient, dependable, demonstrate proficient computer skills, and be able to assist with transfer/lift of a resident up to 100lbs as assistance may be required in positioning of a resident. POSITION QUALIFICATION: Position of Speech Language Pathologist requires a master's degree and ASHA Certificate of Clinical Compliance. Therapist must hold a current license in applicable state(s) where services are being contracted and provided. WORK HOURS: Therapy services are to be provided seven (7) days a week to residents which will include work on the weekends and/or holidays. WORK LOCATION: Position may include work at a facility outside of the facility or facilities listed on your employment offer letter. RESPONSIBILITIES: 1. Administer speech language pathology therapy screening, evaluation, and treatment to residents according to your scope of practice where therapy services are being provided as follows: a) Screen residents to identify individuals who require further evaluation to determine the presence or absence of a communication, cognitive, voice and swallow disorder. b) Upon order of attending physician administer speech, language, cognitive, linguistic, voice and swallowing diagnostic evaluations to resident. c) Obtain informed consent from the resident and/or POA as applicable. d) Perform initial and ongoing evaluations on new admissions, referred residents, and re-evaluations as indicated. Evaluations should summarize the nature of the resident's needs specifying the speech therapy plan of care. Plan of care should include appropriate medical and treatment diagnosis, short- and long-term goals, prognosis, results of evaluation, resident goals, and discharge planning. e) Complete accurate and appropriate daily billing in Optima HCS for residents on caseload. f) Document resident progress in daily treatment encounter notes, weekly progress reports, recertification documentation, and discharge summaries in Optima HCS for each resident in which you provide therapy services. g) Provide ongoing treatment to residents using the most current techniques following appropriate physician orders, ASHA guidelines, and the state practice act's code of conduct/standards. h) Make recommendations for additional assessments as required, i.e. neurology, audiology, neuropsychology, dietary, radiology. i) Follow established procedures for use of physical agent modalities as applicable. j) Participate in discharge planning of residents on caseload and provide rehab discharge programs as applicable. k) Participate in facility care plan meetings, weekly rehab team meeting, company webinars, and interdisciplinary team trainings/conferences. l) Adhere to HIPAA regulations. m) Adhere to the weekly productivity standard of a minimum of 85%. n) Adhere to other delegated tasks on the Evaluating Therapist Task List. 2. Supervise Clinical Fellows: a) Supervise Clinical Fellows as necessary based upon ASHA requirements and State Practice Act. 3. Maintain Licensure and Company Requirements: a) Obtain CEUs per State Board requirements annually/bi-annually. b) Renew licensure prior to expiration. c) Ensure ongoing CPR certification is completed prior to expiration date. d) Ensure annual PPD requirements are completed prior to expiration date. e) Participate in program development, in-service training and educational programs related to Speech Language Pathology.
