FirstLight is seeking LPNs to implement patient-centered geriatric care delivery under the guidance of Director of Nursing; Strong communication, collaboration with team, patient families and critical thinking skills are imperative. LPNs must be comfortable and proficient with electronic medical record documentation. Experience with Axxess software would be helpful. At least two years of hospital, SNF, hospice or home health experience in acute care. Candidates have flexible work days options in daily assignments and have the ability to travel to visit patients in their home setting, focused goal on culture of quality care, patient satisfaction, compliance, self-initiatives and positive work environment.
