FULL-TIME SPEECH-LANGUAGE PATHOLOGIST (SLP)Location: Henry County Health & Rehabilitation Facilities: Paris, TN • Join Our Team! About the Facility Ahava Therapy Services is seeking a Full-Time SLP to join our in-house rehab team at Henry County Health and Rehabilitation Center, a 136-bed skilled nursing facility providing short-term rehabilitation, long-term care, and outpatient services. With a peaceful, family-friendly environment and affordable living, Henry County is an ideal place to grow your career and enjoy a balanced lifestyle. ⚙ Responsibilities Evaluate and treat residents in accordance with Medicare, Medicaid, and commercial payer guidelines. Conduct comprehensive assessments to determine swallowing, cognitive-communication, and language deficits. Develop and implement individualized treatment plans with functional, achievable goals. Document evaluations, daily treatment, progress notes, and discharge summaries accurately and promptly. Revise treatment plans based on patient performance and progress. Educate residents, families, and caregivers on therapeutic techniques and carryover strategies. Collaborate with nursing staff, interdisciplinary team members, and families to maximize functional abilities. Participate in care conferences, facility meetings, and quality improvement activities. Maintain up-to-date knowledge of community resources and assist with discharge planning as needed. Stay clinically current through continuing education and contribute to in-service education. Adhere to professional, ethical, regulatory, and facility standards. Maintain expected productivity levels and demonstrate strong teamwork, communication, and clinical judgment.
