RN Home Health Nurse

Estimated Gross Weekly Pay: $2680 Opportunity: RN - Home Health Job Details: • Work 8-hour day shifts, approximately 8:00 AM to 4:30 PM with potential flexibility based on census • 40 hours per week guaranteed • Possible weekend and holiday call responsibilities • Weekday on-call duties with potential for low census call status • Call back minimum of 2 hours when applicable • Travel to 20-25 patients weekly with 4-5 visits per day on average • Daily travel can vary from 25-60 miles depending on territory and patient census • Total mileage can exceed 100 miles due to large territory coverage • Mileage reimbursement provided at current government rate • Required certifications: BLS (AHA or ARC), active Michigan and Wisconsin RN licenses, valid driver's license • OASIS certification is a plus • Minimum 2+ years home health experience preferred • Travel experience required • Epic charting experience required • OASIS charting experience required • Perform OASIS admissions, all time points, episode management, doctor calls and coordination of care • Complete timely documentation in EMR • Maintain clinical competencies in wound care, IV therapy, ports, lab draws, and chronic disease management • Must demonstrate strong time management, resilience, dependability, and positive attitude • Associate degree or certificate required; bachelor's degree preferred • Contract eligible for 36 hours of unbillable low census hours Facility: This home care and hospice organization provides comprehensive home health services to patients in their communities.
