Join a dedicated healthcare team providing compassionate hospice care in a regional setting within the Midwest. As a Registered Nurse specializing in hospice, you will deliver direct patient care using the nursing process, demonstrating clinical expertise and adhering to professional ethical standards. Essential Duties and Responsibilities Participate in agency councils, committees, or serve as a clinical coach, supporting professional development and quality care. Administer medications and perform planned interventions safely and efficiently, following established policies and protocols. Communicate patient condition changes to physicians clearly and advocate for patient needs using structured communication methods like SBAR. Coordinate patient care collaboratively with healthcare team members and external facilities. Adapt positively to the dynamic healthcare environment and embrace change professionally. Document patient care activities accurately at the point of care, including assessments, interventions, care plans, and physician communications using electronic medical records. Evaluate patient progress toward goals and update care plans accordingly. Develop age-appropriate, patient-centered care plans upon admission with active patient and family involvement. Provide and coordinate education tailored to patient and family needs, considering cultural and educational backgrounds. Engage in self-reflection to learn from experiences and improve performance. Apply critical thinking in assessing patient needs, planning care, and preparing for discharge. Utilize evidence-based practices and maintain up-to-date clinical knowledge and skills. Education Graduation from an accredited nursing school is required. A Bachelor of Science in Nursing is preferred. Experience At least one year of clinical nursing experience is required. Experience in home health nursing is preferred. Certification and License Current RN license in a compact state or the relevant state is required unless you are a new graduate, who must obtain licensure within 90 days of graduation. Basic Life Support certification from the American Heart Association is mandatory. Mental and Physical Requirements This role demands considerable mental focus due to the sensitive nature of end-of-life care. Physical activities include lifting up to 35 pounds, frequent walking, standing, turning, carrying, pushing, pulling, stooping, crouching, twisting, and reaching. Working Conditions Expect frequent exposure to communicable diseases, infectious materials, and potentially hostile situations. Work environments vary and include community settings with exposure to biohazardous materials and temperature fluctuations.
