We are currently seeking a compassionate RN Case Manager to make a meaningful impact by facilitating life-saving care! Role Overview: Experienced RN who is responsible for the overall management of patients, including the provision of direct care with a designated visit productivity standard. This position plans, organizes, and directs all patient care services for patients in the assigned caseload in accordance with current standards and regulations, ensuring the optimal degree of quality care is maintained. Responsible for coordinating the interdisciplinary team to implement the established plan of patient care and ensuring effective management of visit utilization and control of expenses. Requirements: Education: Bachelor's degree in Nursing or Health Administration. Desired: Master's degree in Nursing, Case Management, or related field. Certification & Licensure: Registered Nurse (RN) in California. Certified Case Manager (CCM) preferred. Basic Life Support (BLS) Healthcare Provider certification required. Experience: Minimum of three (3) years in acute medical/surgical/ED or critical care nursing. Consideration for employment with a Master's in Nursing in Case Management instead of three (3) years of acute care experience may be at the director's discretion. Previous Case Management experience preferred. Experience with electronic InteQrual or other standardized criteria strongly preferred.
