The Adolescent Behavioral Health Licensed Practical Nurse contributes to the multidisciplinary team assessment, planning, implementation, and evaluation of nursing care under the direct supervision of a Registered Nurse. Job Responsibilities include, but are not limited to, the following: Utilizes the nursing process to assess, implement and evaluate patient/family care. Provides nursing care (including the administration of medication) to meet patients' needs. Participates in the development and maintenance of a safe and therapeutic milieu in assigned care are. Adheres to and implements organizational and departmental policies and procedures as they pertain to work area. Performs constant observations, intermittent observations, specializing, checks or other forms of patient safety procedures as assigned. Shares information appropriately with member of the multidisciplinary team and other hospital department representatives through verbal reports and written documentation. Participates in pre-admission evaluation as requested by Program Manager and completes Admission Nursing Assessment on all new Admissions to C/A Residential I & Partial Programs (Including ARCC). Conducts monthly medication renewals and reviews daily reports. Responsible for follow up on medical tests and labs. Oversees completion of admission paperwork. Completes medication portion of aftercare forms. Responsible for tracking medical errors.
