Job Requirements / Qualifications Active and up-to-date Registered Nurse (RN) license in the state of Ohio Minimum 1 year of hospice nursing experience within the past 3 years Strong clinical assessment, documentation, and communication skills Ability to work independently and collaboratively with an interdisciplinary team Valid driver's license and reliable transportation to travel within assigned territory Primary Responsibilities Provide compassionate, patient-centered hospice care in homes and facilities across Lorain, Huron, and Erie counties Conduct patient admissions, comprehensive assessments, and develop individualized hospice care plans Manage symptom control and provide education to patients and families on care processes and end-of-life symptom management Collaborate with physicians, social workers, chaplains, and other members of the interdisciplinary care team Document all care, visits, and communication accurately and in a timely manner Support patients and families through end-of-life and palliative care with empathy, professionalism, and clinical expertise This position is ideal for Hospice RNs, Travel Hospice Nurses, and Registered Nurses experienced in home hospice and end-of-life care.
