Mileage Reimbursement: Provided. Premium Pay Rates: $5/hour premium on all hours worked. $1.25/hour weekend shift differential. $1.25/hour evening/night shift differential (6:00 PM - 6:00 AM). Experience Requirements: Long Term Care Experience (6 months preferred, 3 months required). Scheduling Requirements: Minimum of 30 hours per month. One weekend shift per month. Job Summary The Licensed Practical Nurse (LPN) plays a crucial role in delivering professional nursing care to residents of all ages in long-term care settings, under the supervision of a registered nurse, advanced practice provider, or physician. This position requires knowledge of nursing principles, competency in practice, and the ability to meet the physical and psychosocial needs of residents as outlined in their care plans. The LPN ensures resident health, comfort, and safety, contributes to assessments, administers medications or treatments, offers technical support to healthcare professionals, and maintains accurate documentation. Effective communication and relationship-building skills are essential in working with residents, families, and interdisciplinary teams. Qualifications Graduate from an accredited practical nursing program as a Licensed Practical Nurse; an Associate degree in practical nursing is preferred. If IV therapy/management skills are required, completion of an AASPN program or an approved IV Therapy course is necessary. A minimum of 400 hours of experience as an LPN in another facility is required unless the pre-licensure experience has been met. Unencumbered LPN license with the State Nursing board; multi-state licensure privileges are desirable. Completion of annual continuing education and competency training is mandatory.
