The Occupational Therapist will be under the supervision of the Home Health and Hospice Manager. Responsibilities: Accept responsibility for evaluation of the patient's functional status and for interpretations and implementation of the physician's plan of care. Complete assessment/reassessment and make decisions based upon objective tests and measurements. Communicate observations, facts, and treatment suggestions as an interdisciplinary team member, individually and through team meetings, to ensure coordination of the plan of care. Provide education and direction to patients and coworkers as appropriate. Work independently and cooperatively. Recognize and protect patient rights. Convey empathy and compassion to those experiencing grief, pain, or trauma. Complete accurate and concise documentation of all communications, coordination, and treatment activities. Adapt to emergency situations, be alert to changes in the patient's condition. Requirements: Is a graduate of an occupational therapy curriculum accredited jointly by the Committee on Allied Health Education and Accreditation of the American Medical Association and the American Occupational Therapy Association. Licensed by the State of Oregon as an Occupational Therapist. Must have a valid driver's license. Must have a current certification in BLS. Must show proof of auto insurance.
