This role requires high-level clinical decision-making and proficiency in HCHB to maintain detailed records of patient outcomes while meeting daily visit benchmarks. This position is a contract assignment with potential to hire on permanently based upon attendance, performance, and business needs Benefits are available to full-time employees after 90 days of employment and include health, optical, dental, life, and short-term disability insurance A 401(k) with a company match is available for full-time employees with 1 year of service on our eligibility datesRN CASE MANAGER | RESPONSIBILITIES: Patient Assessment & Visits: Perform admission, recertification, routine, PRN, and death visits for hospice patients. Care Coordination: Collaborate with primary care providers, social workers, and discharge planners to implement individualized, comprehensive care plans. Resource Management: Identify and implement non-medical support (such as housing and transportation) to ensure compliance with treatment goals. Clinical Oversight: Continuously reassess cost-efficient, appropriate levels of care and identify opportunities for health promotion and illness prevention. Documentation: Maintain detailed records of clinical, functional, and fiscal outcomes using HCHB. Advocacy: Act as a patient advocate by protecting privacy and ensuring the highest standards of confidentiality.RN CASE MANAGER | REQUIREMENTS: Education: Associateí¢€™s or Bachelorí¢€™s Degree in Nursing (ASN/BSN). Attendance: Commitment to mandatory attendance during the first 90 days. Licensure: Current, unrestricted Pennsylvania Registered Nurse (RN) license. Certification: Valid BLS CPR certification required. Case Management certification is preferred. Experience: Previous Hospice experience is required. Must be comfortable performing admission and recertification visits. Travel: Must have a valid driverí¢€™s license and valid car insurance for travel throughout Lancaster and York counties.
