If interested in this RN Case Manager position APPLY NOW for IMMEDIATE considerationBrittney Blackman | | text 5867107970RN CASE MANAGER | DETAILS AND COMPENSATION: Location: York, PA í¢€“ 100% On-site, home health settings Counties Covered: Lancaster and York Payrate: $56.14/hr + mileage Required Availability: Full-Time | Mondayí¢€“Friday, 8:00 AM í¢€“ 4:30 PM Productivity: Expected to complete 5 to 7 visits per day. no weekend or on-call RN CASE MANAGER | SUMMARY AND HIGHLIGHTS: The RN Case Manager willcollaborate with multidisciplinary teams to implement individualized care plans, ensuring all medical and non-medical support systems are in place to optimize patient comfort and dignity. This role requires high-level clinical decision-making and proficiency in HCHB to maintain detailed records of patient outcomes while meeting daily visit benchmarks. This position is a contract assignment with potential to hire on permanently based upon attendance, performance, and business needs Benefits are available to full-time employees after 90 days of employment and include health, optical, dental, life, and short-term disability insurance A 401(k) with a company match is available for full-time employees with 1 year of service on our eligibility datesRN CASE MANAGER | RESPONSIBILITIES: Patient Assessment & Visits: Perform admission, recertification, routine, PRN, and death visits for hospice patients. Care Coordination: Collaborate with primary care providers, social workers, and discharge planners to implement individualized, comprehensive care plans. Resource Management: Identify and implement non-medical support (such as housing and transportation) to ensure compliance with treatment goals. Clinical Oversight: Continuously reassess cost-efficient, appropriate levels of care and identify opportunities for health promotion and illness prevention. Documentation: Maintain detailed records of clinical, functional, and fiscal outcomes using HCHB. Advocacy: Act as a patient advocate by protecting privacy and ensuring the highest standards of confidentiality.RN CASE MANAGER | REQUIREMENTS: Education: Associateí¢€™s or Bachelorí¢€™s Degree in Nursing (ASN/BSN). Attendance: Commitment to mandatory attendance during the first 90 days. Licensure: Current, unrestricted Pennsylvania Registered Nurse (RN) license. Certification: Valid BLS CPR certification required. Case Management certification is preferred. Experience: Previous Hospice experience is required. Must be comfortable performing admission and recertification visits. Travel: Must have a valid driverí¢€™s license and valid car insurance for travel throughout Lancaster and York counties.
