As a Primary Care Nurse (RN), you will apply your extensive training to provide personalized care in a rewarding setting that fosters autonomy. Key Responsibilities: Perform comprehensive assessments to create individualized care plans. Promote effective communication between patients and the interdisciplinary team. Build and maintain therapeutic relationships through education, advocating for patient independence. Utilize advanced technology for documenting skilled services provided. Qualifications: Current Registered Nurse with a valid CT RN license. Minimum one year of RN experience; homecare experience preferred. Valid driver’s license and reliable transportation. Proficiency with computers is required.
