Job Qualifications / Job Description Position Summary Responsible for assessing, coordinating, and managing long-term services and supports (LTSS) for members with complex medical needs. Develops individualized care plans, conducts assessments, ensures service delivery without gaps, and collaborates with providers and caregivers to promote quality and cost-effective healthcare outcomes. Key Responsibilities Conduct comprehensive in-home assessments Develop and manage individualized care plans Coordinate services and referrals Monitor service delivery and member progress Ensure documentation compliance within required timelines Collaborate with providers, caregivers, and interdisciplinary teams Support members with complaints and appeals as needed Education / Certification / License Requirements Education Bachelor's Degree in Nursing or related healthcare field (ASN accepted where applicable) Licensure Active RN License (State or Compact) and Valid Driver's License Experience 2-6 years of clinical or care management experience (Hospital CM, Home Health, LTSS preferred) 5 Benefits of Travel Nursing A diverse range of assignments on your resume showcases your adaptability, versatility, and resilience.
