This role requires an autonomous clinician capable of managing a complex caseload and performing high-level clinical assessments within the community. Non-Negotiable Required Qualifications: Please note: Candidates who do not meet 100% of the following criteria will not be considered for this role. Experience: A minimum of 3 years of professional nursing experience is strictly required. Clinical Expertise: Mandatory, verifiable experience in Home Health nursing and Case Management. OASIS Proficiency: Mandatory expertise in OASIS documentation (SOC, Recertification, Discharge) is required for this role. Licensure: Must hold an active Vermont RN License or a valid Multi-State Compact License at the time of application. Certifications: All professional certifications (e.g., BLS) must be active and in good standing. Schedule Commitment: This is a 20-week contract with NO Requested Time Off (RTO) allowed. Travel Experience: Strong preference is given to experienced travelers who can hit the ground running with minimal orientation. Key Responsibilities Coordinate and manage patient care plans for a dedicated community territory. Perform comprehensive OASIS assessments and maintain real-time EMR documentation. Provide skilled nursing interventions including wound care, medication management, and patient education.
