Growth Opportunities: Supportive team culture and continuing education Your Role:Provide one-on-one Physical Therapy services to patients in their homes. Assess patient functioning levels and develop individualized treatment programs. Educate patients and caregivers on therapy programs and equipment use. Supervise Physical Therapy Assistants and collaborate with healthcare teams. Prepare reports and contribute to agency development initiativesQualificationsMaster's Degree or Higher in Physical TherapyAt least 1 year of experience in a healthcare setting (No new grads). Valid Illinois (IL) PT License & Drivers LicenseHome Health experience preferred, but inpatient rehab experience will be consideredMust have either Home Health or inpatient PT experience; candidates without this will not be interviewedWork Schedule & Travel Details:Monday to Friday, daytime shifts with start & end times varying based on patient needs1 weekend every 6 weeks (Saturday & Sunday, day shift with scheduled patients)2 holidays per year (1 summer, 1 winter on a rotating basis)Local travel required to patients' homes in Elmhurst, Oak Park, Cicero, Maywood, Bellwood, La Grange, La Grange Park & surrounding areas Application Process & Requirements:Completion of the attached interview prescreen is REQUIRED.
