Cardiology Nurse Navigator

At SSM Health Cardinal Glennon Children's Hospital, we are seeking a dedicated RN-Cardiology Nurse Navigator to provide exceptional patient care and guidance throughout the healthcare journey. Job Highlights: Department: Cardiology Schedule: Part Time, Day Shift, 20 hours a week. Work schedule is Monday through Friday, with hours of either 8:00 AM to 4:30 PM or 8:30 AM to 5:00 PM. You’ll alternate between working 3 days one week and 2 days the next. Experience: Requires 3 years of licensed RN experience. Ideal Candidate: You are exceptional with teamwork, flexible, adaptable, and possess strong interpersonal and communication skills. Job Summary: As a Nurse Navigator, you will guide patients through the continuum of care, ensuring they receive the necessary support and resources. Key Responsibilities: Act as a liaison among management staff, administration, physicians, managed care companies, community organizations, and patients. Collaborate with healthcare providers to guarantee quality patient outcomes, facilitating treatment initiatives and educational programs. Gather and analyze data, track patient outcomes, and recommend improvements for care delivery processes. Provide referrals and support for follow-up care for patients without insurance or underinsured individuals. Utilize the nursing process (assessment, planning, intervention, evaluation) in all patient interactions. Assist medical providers with diagnostic results, highlighting critical findings that impact patient care. Understand medication dosages and interactions, providing patient education about medication therapy. Establish and maintain relationships focused on patient education, family counseling, and public information dissemination. Maintain a constant state of alertness and safety while performing duties. Complete other tasks as assigned. Education: Graduate of an accredited school of nursing. Physical Requirements: Constant use of speech and communication skills. Frequent standing, walking, sitting, and use of a keyboard/data entry. Occasional lifting/moving of patients and objects weighing up to 50 lbs. Certifications: Current Basic Life Support HealthCare Provider (BLS HCP) certification from the American Heart Association. Registered Nurse (RN) license in the state of Missouri or issued by a Compact State. Location: SSM Health Cardinal Glennon Children's Hospital.
