POSITION SUMMARY The Occupational Therapist (OT) is responsible for empowering home health patients to participate more fully in daily tasks when this ability is compromised by a health condition. Occupational therapy involves skilled evaluation, planning, and treatment by helping patients learn or re-learn the tasks that are necessary for healthy, meaningful, and day-to-day living at home. The OT is responsible for identifying these patient needs and establishing a plan for optimizing self-care during the home health service episode. These include - management skills related to basic self-care, rest and sleep, social participation, health management, including medication management, eating, physical activity, coping with symptoms, communication, and overall maintenance of wellbeing. Work schedules are determined to meet patient care needs and work may be required on weekends and holidays. This job description applies to both scheduled and per-diem Occupational Therapist roles. The minimum scheduled role is 0.5 FTE and the minimum work expectation for a per-diem role is one (1) shift/month. ESSENTIAL JOB KNOWLEDGE Evaluates and re-evaluates patients using knowledge of the principles and methods of patient assessment and on-going reassessment to develop and modify effective assessments and treatment plans. Selects appropriate assessment tests and measures to conduct patient assessment and reassessment. Incorporates prioritized impairments to establish a therapy classification or therapy diagnosis that is specific to the patient. With patient and family input, establishes goals that reflect anticipated outcomes. Provides and progresses therapeutic interventions to patients that are safe, timely, effective, efficient, patient-centered, and equitable, in compliance with organizational policies and procedures. Practices competently in accordance with professional practice standards and expectations. Recognizes cultural factors that may impact treatment and plan of care and accesses appropriate resources to meet cultural needs and maximize patient outcomes. Facilitates patient responsibility for self-care. Demonstrates a commitment to continued learning by attending continuing education, reviewing the literature, and seeking other resources. As opportunities exist, actively participates in student clinical education. Successfully completes initial and ongoing organizational and department education, training, and competency requirements. Infection Control: Understands, demonstrates, and follows organizational infection control guidelines (CDIC). Quality: Focuses on improving processes and champions efficiency in daily work activities. Safety: Promotes a culture of safety by taking accountability for individual actions, including but not limited to, promptly reporting errors or near misses via the organization's occurrence reporting system and participating in developing improved safety processes, as appropriate. (ADM015, ADM016, OPS044, Mission Statement). Performs other duties as assigned. JOB REQUIREMENTS Licenses: State of Wisconsin Driver's License required. State of Wisconsin Professional licensure for Occupational Therapist required. Education: Bachelor's degree in occupational therapy required. Master's degree in occupational therapy preferred. Experience: A minimum of one (1) year of recent experience in Occupational Therapy required. Experience and strong knowledge base in home healthy therapy preferred. Miscellaneous: Familiarity with variety of equipment, including computer software preferred. Organization, time management, flexibility, and a positive attitude are crucial to a successful performance. CPR/BLS Certification required. Valid Auto Insurance required. Reliable vehicle transportation required.
