Our interdisciplinary team works closely with patients and families to ensure comfort, dignity, and support throughout the hospice journey.We are currently seeking a dedicated and compassionate Part-Time Licensed Master Social Worker (LMSW) to join our growing team in the DFW Metro area.Position SummaryThe Hospice Social Worker provides essential psychosocial support to hospice patients and their families. This role focuses on counseling, care coordination, resource connection, and emotional support during end-of-life care.Hours are flexible and based on the needs of the assigned patient caseload.Key ResponsibilitiesConduct psychosocial assessments for hospice patients and familiesProvide emotional support, counseling, and crisis intervention as neededAssist patients and caregivers with advance care planning and end-of-life resourcesCollaborate with the interdisciplinary hospice team to develop and implement the plan of careProvide education and support related to grief, loss, and coping strategiesConnect families with community resources, financial assistance, and support servicesParticipate in care conferences and documentation per hospice regulationsMaintain accurate, timely clinical documentation in compliance with Medicare and hospice standardsQualificationsActive LMSW license required (Texas)Master’s degree in Social Work (MSW)Previous hospice or healthcare experience preferredStrong interpersonal, communication, and organizational skillsCompassionate approach to supporting patients and families during end-of-life careAbility to work independently and manage a flexible caseload scheduleReliable transportation for travel within the DFW Metro areaWhat We OfferFlexible part-time scheduleSupportive interdisciplinary team environmentMeaningful work serving patients and families during critical momentsCompetitive compensation based on experienceHow to ApplyIf you are passionate about hospice care and making a difference in the lives of patients and families, we encourage you to apply.Please submit your resume to:
