Admission Nurse (LPN)

TLC Health & Rehab is seeking a Full-Time Admissions Nurse to join our team! The Admissions Nurse plays a key role in welcoming new residents and ensuring a smooth and efficient admission process. This position works closely with the clinical team, admissions department, and families to coordinate resident admissions and provide excellent customer service. Position: Admissions Nurse Status: Full-Time Location: TLC Health & Rehab Responsibilities include: • Coordinating and completing resident admissions • Reviewing hospital referrals and clinical documentation • Communicating with hospitals, families, and internal teams • Completing admission assessments and required paperwork • Ensuring a positive admission experience for residents and families Qualifications: • Active RN or LPN license • Skilled Nursing Facility (SNF) experience preferred • Strong clinical assessment and communication skills • Ability to work collaboratively with interdisciplinary teams If you are an organized nurse who enjoys working with residents and families during the admission process, we would love to hear from you.
