Health Aide

Join Our Caring Community and Help Us Put Health First As a Health Aide (HHA), you will play a vital role in providing compassionate, direct patient care under the guidance of a Registered Nurse, according to the Aide Plan of Care (POC). Your responsibilities will include: Supporting patients with self-administered medications, ensuring they take the correct doses safely. Taking and recording vital signs such as temperature, pulse, and blood pressure, and promptly reporting any abnormal results. Assisting patients with personal hygiene, including bathing, grooming, and oral care. Helping with meal times by observing and recording nutritional intake and assisting with feeding as required. Providing assistance with toileting and related care needs, ensuring comfort and dignity for the patient. Facilitating safe mobility for patients, including exercises, transfers, and ambulation as outlined in the Care Plan. Documenting patient interactions accurately and thoroughly to ensure continuity of care. Maintaining a clean and safe environment while adhering to infection control principles. Assisting with light household duties like laundry, meal prep, and housekeeping when needed. Required Experience/Skills: High school diploma or equivalent. Completion of a Certified Nursing Assistant or Certified Home Health Aide Program within the last 24 months. Compliance with applicable state certification requirements. A valid driver's license, auto insurance, and reliable transportation are required. Good standing on the HHA Registry (if applicable) with completion of the necessary HHA/CNA course. At least one year of experience in the last 24 months as a Home Health Aide or Certified Nursing Assistant in relevant healthcare settings.
