The ideal candidate will be responsible for assessing, planning, and implementing rehabilitative programs to improve patients' mobility, reduce pain, and enhance physical function. Duties include evaluating patient conditions, developing individualized treatment plans, utilizing therapeutic exercises and modalities, and documenting patient progress in compliance with Oregon state regulations. Key Responsibilities: Conduct patient evaluations and create customized treatment plans. Administer physical therapy interventions, including exercises, manual therapy, and equipment use. Monitor and document patient progress, adjusting treatments as necessary. Educate patients and families on recovery processes and home exercise programs. Ensure adherence to Oregon licensing standards and professional ethics. Qualifications: Licensed in Oregon or a graduate of an approved Physical Therapist program or actively working toward licensure. Graduate of a CAPTE Accredited Physical Therapy Education Program Experience in post-acute care or similar healthcare environment preferred Excellent oral and written communication skills Demonstrates effective problem-solving skills Ability to collaborate with facility staff to promote highest practicable physical, mental and psychosocial wellbeing.
