Hospice Triage RN (Weekend Nights)

Position Details: Main Office location: Londonderry, NH Service Area: Londonderry & Surrounding areas Job Type: Part-time Schedule: Saturday and Sunday 8:00 pm - 8:30 am Responsibilities: Initiates appropriate preventive and rehabilitative nursing procedures. Administers medications and treatments as prescribed by the physician. Initiates the plan of care and makes necessary revisions as patient status and needs change. Responds promptly to incoming calls during afterhours. (Weekends 8:30 AM to 8:00 PM) Assesses patient's needs through triaging and providing in-home care when needed. Collaborates with physicians, nurse practitioners and interdisciplinary team to contribute a strong Plan of Care for each patient. Responds to inquiries and concerns from patients, their caregivers, and their medical team. Consults patients and caregivers before, during and after treatment. Qualifications: 2+ years' nursing experience, preferably in geriatrics is desirable. New nurses, feel free to apply! We have a great training program. Degree in Nursing from an accredited institution. Proficient with electronic medical records. Excellent bedside manner and communication skills. Working knowledge of health care laws and regulations. Maintain current BLS CPR certification. Possess a valid driver's license and own a reliable, insured vehicle. Benefits & Compensation: Pay Structure: Base Compensation ($600 per weekend) + Paid per visit ($50) and per admission ($100). Visits over 60 minutes are paid at $75. Training and meetings are paid hourly at $35/hr. Mileage Reimbursement ($0.36/mile) 401(k) Match.
