About the Role As a Speech Language Pathologist at LifeSpan Therapy, you will play a critical role in assessing, diagnosing, and treating individuals with speech, language, communication, and swallowing disorders. Your work will directly impact the quality of life for residents by helping them improve their communication abilities and overall functional independence. You will collaborate closely with a multidisciplinary team to develop personalized treatment plans tailored to each resident's unique needs. This role requires a compassionate approach to resident care, ensuring that therapy sessions are both effective and supportive. Ultimately, your expertise will contribute to the advancement of resident outcomes and the enhancement of their social and emotional well-being. Job Responsibilities Conduct comprehensive evaluations of residents' speech, language, cognitive-communication, and swallowing abilities. Coordinates admission/discharge to speech therapy with the Interdisciplinary Team, Restorative Care Nurse and staff nurses. Provides therapy to residents under specific medical orders and in collaboration with the Interdisciplinary Care Plan Team. Monitor resident progress and adjust therapy techniques as necessary to achieve optimal outcomes. Educate residents, families, and caregivers on communication strategies and therapeutic exercises to support ongoing improvement. Maintain accurate and timely documentation of resident evaluations, treatment plans, and progress notes in compliance with regulatory standards. Collaborate with healthcare professionals, including physicians, occupational therapists, and physical therapists, to coordinate resident care. Minimum Qualifications Must be a licensed therapist who is a graduate of an accredited school. Must have a valid Indiana Operator's license and be able to travel occasionally to other locations if needed to provide therapy coverage. Basic computer literacy for documentation and communication purposes. Preferred Qualifications Experience working in outpatient or rehabilitation therapy settings. Familiarity with electronic health record (EHR) systems. Strong interpersonal skills with experience in resident and family education.
