Every other weekend required; holidays as required.In this position, you will collect current and accurate information from patients and families with respect to prescribed home medications for the patient. You will then enter the data into the electronic medical record (EMR) and produce an admission medication reconciliation sheet to be presented to a pharmacist for review, and subsequently made available for physician review.Principal Duties and Responsibilities:Collect information regarding all current home medications from patients or caregivers, primarily in the Emergency DepartmentEnter patient medication information into the EMRProduce documents for review by pharmacist and subsequent physician reviewProvide assistance to healthcare providers to facilitate the provision of medicationsNavigate the ED tracking board to determine patient selectionSolicit other sources of information to verify accuracy of collected dataDetermine time last dose of each medication was taken, medication compliance prior to admission, and recent antibiotic medications presecribedObtain medication allergy history and related reactions to the medicationOrient and train new pharmacy employeesEducation and Experience Requirements:High School diploma or equivalent required; Associate or Bachelors degree preferredPrevious hospital pharmacy experience preferredApplicants enrolled in, or graduate of a 4-year bachelor program in the following health science majors are encouraged to apply and will be required PTCB certification in 1 year of employment: Applied Molecular Biology and Biotechnology Biology, Chemistry, Exercise Science, Health Behavior Science, Human Physiology, Medical Diagnostics, Medical Diagnostics Pre-PA, Medical Laboratory Science, Nutrition, Nutrition and Dietetics, Nutrition and Medical Sciences, Nursing, Occupational Therapy, Physical Therapy, Physician Assistant or Sports HealthMaryland Pharmacy Technician License requiredPharmacy Technician Certification Board certification (PTCB) requiredComputer experience preferred (e.g., window-based programs)ChristianaCare Offers:Incredible Work/Life benefits including medical/dental/ vision insurance coverage on your first day of employment, paid PTO, retirement plans, twelve weeks of paid parental leave, annual membership to , access to backup care services for dependents through Care@Work, retirement planning services, financial coaching, fitness and wellness reimbursement, and great discounts through several vendors for hotels, rental cars, theme parks, shows, sporting events, movie tickets and much more.If you 're inspired to make a difference, we invite you to become a ChristianaCare caregiver!Hourly Pay Range: $19.84 - $29.76This pay rate/range represents ChristianaCare's good faith and reasonable estimate of compensation at the time of posting.
