Remote Care Navigator – Senior & Dementia Care We’re seeking a compassionate and experienced Care Navigator to join a mission-driven healthcare team supporting family caregivers nationwide. This fully remote position focuses on empowering those caring for seniors and individuals living with dementia through education, coordination, and emotional support. Role Overview As a Care Navigator, you’ll serve as a trusted partner to caregivers, helping them access non-clinical resources such as Medicare, VA, employer-sponsored programs, and local community services. You will be the main point of contact and problem-solver for caregivers — part coach, part resource expert, and part navigator. Your job is to listen, guide, connect, and follow up so caregivers don’t have to figure everything out alone. Key Responsibilities Conduct phone, video, and chat consultations with caregivers to assess needs and identify available resources Develop and document individualized action plans within 24 hours of each interaction Collaborate with care teams, social workers, and case managers to maintain alignment Provide ongoing education and guidance, including dementia care techniques and community resource navigation Proactively re-engage caregivers who may need additional support Qualifications 2 years of professional experience in senior care, dementia care, or geriatrics At least 2 full years of previous remote work experience required Degree in Social Work, Nursing, Case Management, or Human Services preferred (equivalent experience accepted) Strong communication, empathy, and organizational skills Comfortable using technology to support clients remotely Bonus: bilingual (Spanish or Mandarin), dementia-specific certification, or experience serving diverse populations Schedule Fully Remote | 4-day, 10-hour schedule (4x10) Monday & Friday required; remaining days fixed once assigned Time zone preferred: CST or PST Example schedule: 9 AM–7 PM CST or 7 AM–5 PM PST What We Offer 100% Remote Work Environment Additional Day Off Each Week Training Toward Dementia Practitioner Certification (within 90 days) Meaningful, mission-driven work improving caregiver well-being If you have a heart for caregiving, proven remote experience, and a passion for supporting families navigating senior care—apply today to make a lasting impact.
