Care Manager - North Haven, CT Agency on Aging of South Central Connecticut (AOASCC) Make a meaningful impact every day. Position Summary As a Care Manager at AOASCC, you'll play a vital role in supporting older adults as they navigate life's transitions. You'll help clients live safely and independently by developing individualized care plans, coordinating key services, and working hand-in-hand with families, healthcare professionals, and community partners. Key ResponsibilitiesDevelop and manage personalized care plans based on client assessments and needsCoordinate services including hospital discharge planning, mental health support, and long-term care optionsCollaborate with family members, healthcare providers, and interdisciplinary teams to ensure comprehensive, seamless careAdvocate for clients and connect them to appropriate community-based resources and supportsMaintain thorough, timely documentation in compliance with agency and regulatory standards Minimum QualificationsBachelor's degree in Human Services, Social Work, or a related fieldAt least 2 years of experience in case management or a similar social services roleStrong understanding of Medicaid regulations and care coordination practicesExcellent communication, organizational, and interpersonal skillsCompassionate, patient, and committed to a client-centered approach Preferred QualificationsMaster's degree in Social Work or a related disciplineExperience working with older adults in home care or community-based settingsCertification in Case Management (CCM, ACM, or equivalent) Key SkillsEmpathy and high emotional intelligenceStrong relationship-building and active listening abilitiesSolid knowledge of elder care systems and MedicaidDetail-oriented with excellent time managementProven success in coordinating long-term care services Why Join AOASCC?
