An opportunity is available for an Occupational Therapist specializing in home health services, working within a hospital therapy setting. This contract position offers a Monday through Friday schedule, 8 a.m. to 5 p.m., with rotating weekends, providing a consistent yet dynamic work routine. Key Responsibilities: Deliver skilled occupational therapy to patients in a home health environment. Assess patient needs and develop individualized treatment plans. Coordinate care with interdisciplinary teams to support optimal patient outcomes. Document patient progress and manage care plans efficiently. Qualifications and Experience: Minimum of one year of experience practicing as an Occupational Therapist. Proven background in home health occupational therapy services. Current Illinois OT license and Basic Life Support (BLS) certification. Flexible and reliable transportation; mileage reimbursement is provided for travel between patient visits. No local candidates available within a 50-mile radius, indicating travel may be necessary.
