Job Description & Requirements Specialty: Home Health Discipline: RN Duration: 13 weeks 40 hours per week Shift: 8 hours, days Employment Type: Local Contract Must have a Associate's Degree in Registered Nurse specialization Must have a High School Diploma Good to have a Bachelor's Degree in Registered Nurse specialization Must have 0-1 years of experience in Provide quality nursing care for patients in home setting Must have 0-1 years of experience in Bilingual English/Russian Nice to have knowledge in Assess patient, families, and caregivers' ability to manage patient care. Teach patient, families, and caregivers' in the care of long/short term illness and wound care Nice to have knowledge in Determine intermittent visits based on acuity and level of care needed. Nice to have knowledge in Complete re-certifications, resumption of care, transfers, and discharges. Nice to have knowledge in Consult with and educate patient and family regarding the disease process, self-care techniques, and prevention strategies. Nice to have knowledge in Coordinate all patient/family services and prioritization of needs with the members of the interdisciplinary team: OT, PT, MSW, and speech therapist.
