We are currently seeking Family Medicine, Internal Medicine, or Geriatrics providers to join a PACE (Program of All-Inclusive Care for the Elderly) setting in Cambridge, MA. This opportunity is open to Physicians, Nurse Practitioners, and Physician Assistants who are passionate about caring for complex older adults and medically fragile patients. The PACE model focuses on coordinated, patient-centered care, allowing providers to work closely with interdisciplinary teams to improve the health and quality of life of elderly participants. Position Highlights Work in a unique PACE care model focused on elderly and complex patients Opportunity for Physicians, Nurse Practitioners, and Physician Assistants Balanced weekday schedule with no call responsibilities Collaborative interdisciplinary care team environment Utilize Epic EMR for seamless documentation and patient management Schedule Assignment Dates: May 1, 2026 November 30, 2026 Schedule: Monday Friday Hours: 8:00 AM 5:00 PM Call: No call required This schedule allows for a consistent work-life balance while delivering high-quality patient care. Clinical Responsibilities Provide comprehensive primary care for PACE program participants Manage chronic and acute medical conditions Conduct clinic visits, home visits, and nursing home visits as assigned Participate in interdisciplinary care team meetings Coordinate care with nursing, social workers, therapists, and support staff Manage patient communications such as phone and in-basket coverage Document patient encounters using Epic EMR Patient Population PACE programs serve elderly patients and individuals with complex medical conditions who require coordinated long-term care. Typical Patient Panel: Approximately 70 patients Patient volumes are smaller due to higher complexity of care This allows providers to focus on quality care and comprehensive patient management. Practice Environment Work alongside physicians, nurse practitioners, and physician assistants Strong interdisciplinary care teams including nurses, therapists, and social workers Combination of clinic, home, and nursing facility care settings Epic EMR system Requirements MD / DO Family Medicine, Internal Medicine, or Geriatrics (for physicians) Nurse Practitioner or Physician Assistant certification (for APPs) Board Certified (for physicians) Active Massachusetts Medical License required Flu vaccination required (no waivers accepted) Recent direct supervisor reference required Why Cambridge, MA?
