Job Responsibilities: Conduct comprehensive assessments to create individualized care plans. Facilitate communication between patients and the interdisciplinary team. Develop and maintain therapeutic relationships by providing education and encouraging self-advocacy and independence. Utilize modern technology to document the skilled services provided. Qualifications: Registered Nurse with a valid CT RN license. Minimum one year of RN experience; homecare experience is a plus. Valid driver's license and reliable transportation are mandatory. Computer proficiency is required.
