Job Summary: As a Graduate Nurse, you will provide direct nursing care in alignment with our established policies, procedures, and protocols, impacting patient outcomes positively. Key Responsibilities: Implement and monitor patient care plans while recording and communicating patient conditions appropriately. Act as the primary coordinator among all healthcare disciplines to ensure cohesive patient care. Execute physician and nursing orders consistently and accurately. Assess and coordinate patient's discharge planning needs with the healthcare team. Demonstrate the skills necessary to assess patients and address their physical, psychosocial, spiritual, and age-related needs. Delegate care responsibilities to other nursing staff based on their qualifications and competencies. Engage in daily interdisciplinary patient rounds, providing relevant insights to meet patient goals. Uphold the ANA Standards of Nursing Practice and Code of Ethics. Develop, implement, and update individualized care plans based on patient needs. Participate in professional growth activities outside of direct patient care. Perform additional duties as assigned. Qualifications: Graduate of an accredited nursing school. Associate degree required; BSN preferred. Current Connecticut RN license. BLS certification from the American Heart Association. Strong assessment and critical thinking abilities.
