Maintains the standards of professional nursing. Supervises CNA and coordinates care with other health care workers as assigned. Provides comprehensive initial assessments to all clients and on subsequent visits continually assesses Physical, Psychosocial, Environmental and community resources available utilizing the principles of growth and development over the life span Initiates Plan of Care, re-evaluates the need for care and modifies Plan of Care as ordered / needed. Establishes discharge plans with patient, physician, other disciplines and caregivers. Completes OASIS data Provides patient care using current Federal, State and Agency regulations Assesses educational needs of patient, families, caregivers and implements appropriate teaching strategies Completes documentation according to regulatory guidelines. Promotes a clean and safe environment of care, utilizing the SAFE error prevention habits Provides the highest standard of privacy and confidentiality in matters involving patients, coworkers, and the hospital by abiding by the Standards of Conduct Submission Requirements Home Health experience - REQUIRED Valid drivers license and car insurance - REQUIRED Copy of agency's IL nursing license included in submission - REQUIRED Will consider someone with strong Med Surg experience with some Home Health experience COVID and FLU vaccination required - DECLINATIONS WILL BE APPROVED Please add vaccination status in the profile OASIS and start of care experience - strongly preferred Manager is wanting to review local candidates! Territory: Within 15 miles (may increase if census is low and they need to offer the 40 hours) Census: 24-26 on case load. If given a bigger territory that number will go down. Mileage: Reimbursed at the standard IRS rate. Perform some Telehealth/Virtual visits in addition to home visits Orientation: 2 days. First day will be in the office. Second day out in the field.
