Productivity requirement of 30í¢€“32 points per week. HOME HEALTH PHYSICAL THERAPIST | SUMMARY AND HIGHLIGHTS: The Home Health Physical Therapist will perform comprehensive home-based evaluations and treatments to help patients regain functional mobility and independence following physician referrals. This role involves developing measurable rehabilitation goals, supervising PTAs, and securing adaptive equipment while documenting progress within a multi-county service area. This position is a contract assignment with potential to hire on permanently based upon attendance, performance, and business needs Benefits are available to full-time employees after 90 days of employment and include health, optical, dental, life, and short-term disability insurance A 401(k) with a company match is available for full-time employees with 1 year of service on our eligibility dates HOME HEALTH PHYSICAL THERAPIST | RESPONSIBILITIES: Perform comprehensive physical therapy evaluations and assessments based on physician referrals. Review patient medical history, including precautions and contraindications, to ensure safe treatment. Establish measurable goals and develop specialized treatment plans to improve patient function. Regularly reassess treatment effectiveness and modify goals or plans as patients progress. Communicate patient needs and progress to the treatment team, physicians, and family members. Recommend and assist in securing adaptive equipment to enhance the rehabilitation program. Supervise Physical Therapist Assistants (PTAs) and PT Aides in accordance with the state practice act. Maintain accurate and timely clinical and billing documentation in compliance with state and federal regulations. HOME HEALTH PHYSICAL THERAPIST | REQUIREMENTS : Education: High School Diploma or GED (verifiable) Attendance: Commitment to mandatory attendance during the first 90 days. License: Active Alabama Physical Therapist License (Compact licenses are accepted). Adherence to HIPAA, Corporate Compliance programs, and professional practice standards. Preferred Skills Previous experience in a Home Health Proficiency with HCHB (Homecare Homebase) EMR software. Strong communication skills for coordinating with medical providers and families.
