Position Summary: As a Registered Nurse in our Cardiac Rehabilitation department, you will engage in the nursing process—assessment, planning, intervention, documentation, and evaluation—to provide tailored patient care. You will create individualized education and care plans based on the unique needs of patients and their families. You will focus on maintaining confidentiality and being sensitive to the comfort and privacy of patients. Essential Functions: Assess and identify patient and family needs to ensure high-quality care. Develop and communicate individualized care plans in collaboration with the healthcare team and patients/families. Implement medical care plans and nursing interventions with skill and judgment. Continuously evaluate patient responses and adjust care plans as needed. Maintain accurate documentation to support coordinated, quality care. Keep certifications and licensure current and engage in ongoing professional development. Serve as a role model for colleagues. Follow safety standards, including Exposure Control Plans for bloodborne and airborne pathogens. Demonstrate respect and professionalism toward patients, families, visitors, and colleagues. Promote effective teamwork within and across departments. Uphold CHH core values: dignity, compassion, service excellence, community, and integrity. Participate in quality and performance improvement initiatives. Qualifications: Active Connecticut RN license. Associate degree required; Bachelor's degree preferred. Required certifications: ACLS, BLS. 2 years of Cardiac or Critical Care experience preferred. We value your career.
