About the Role As a Nurse Practitioner with Matrix, you will conduct thorough assessments that provide deep insight into members' health during visits that last up to one hour. Your assessments will help identify and close care gaps effectively. Your key responsibilities include: Conducting adult/geriatric assessments, including medical history, diagnosis, health education, case management referrals, physician referrals, and documentation per Matrix protocols. Administering and ordering necessary point-of-care and lab-based testing. Collaborating with physicians, case managers, social workers, families, key caregivers, and other medical professionals. Partnering with Primary Care Physicians for patient education and follow-ups as needed. Providing services in the member's residence or Skilled Nursing Facilities. Benefits of Working at Matrix Flexibility : Schedule visits based on YOUR availability from Sunday to Saturday between 8 am and 7 pm, allowing you to work during the day, evenings, or weekends. Competitive Pay : Earn income according to the number of assessments completed, with the potential to earn between $1,800 and $11,500 per month based on assessment rates of $90 to $115. Skills & Experience That Shine A Master's Degree or equivalent experience, along with valid NP licensure. Current NP licensure in your state of practice, including prescription authority. Board certification with AANP, AACN, or ANCC in relevant specialties. Current BLS, ACLS, or CPR certification. Preferred: 1 year of NP experience; new graduates are encouraged to apply! A valid state driver's license and automobile insurance as per your state's requirements. Strong computer skills with familiarity in health/medical record software.
