Our Certified Nursing Assistants (CNAs) play a vital role in bringing this mission to life by providing essential, hands-on care and meaningful support to those we serve. We are looking for CNA's in Conroe/Montgomery, TX and surrounding areas. About the Role: As a Home Health CNA with Pathfinder, you’ll provide personal care and daily living assistance under the guidance of a Registered Nurse (RN) and in accordance with each patient’s individualized care plan. You’ll work one-on-one with patients in their homes—helping them maintain independence, dignity, and comfort—while being supported by a collaborative, responsive clinical team.
