About the Role: As a Home Health CNA with Pathfinder, you’ll provide personal care and daily living assistance under the guidance of a Registered Nurse (RN) and in accordance with each patient’s individualized care plan. You’ll work one-on-one with patients in their homes—helping them maintain independence, dignity, and comfort—while being supported by a collaborative, responsive clinical team.
