For more information, visit Job Title: Advanced Practice Provider - Ortho (Total Joint) Requisition Number: req27574 Location: John Peter Smith Hospital Job Description:Job Summary: The Advanced Practice Provider - Ortho (Total Joint) is responsible for providing and coordinating care for total joint replacementpatients. The Advanced Practice Provider will workalongside and directly support the orthopedic surgeon in the OR, hospitalrounds, and clinic. In addition to assisting with all aspects of patient care, the Advanced Practice Provider will assist in the planning, development, andimplementation of program goals and operations. The Physician Assistant - Orth (Total Joint) coordinates and oversees the ongoing disease-specific certificationprocess to continually improve care for the orthopedic patient population. Duties and Essential Job Functions: 1. Coordinates the efforts between the surgeons, JPSleadership and staff while tracking the progress of the program and identifyingareas requiring attention. 2. Lead efforts to gain and maintain the organization's DNVTotal Hip and Knee Replacement Certification. 3. Responsible for all program management, reportgeneration and review, process improvement, patient and provider satisfaction. 4. Works collaboratively with joint arthroplasty surgeonsand anesthesiologists to manage the patient's specialized needs. Coordinatesthe interdisciplinary approach to providing continuity of care, including utilizationmanagement, discharge coordination and planning, and obtaining all authorizations/approvals/transfersas needed for outside services for patients/families. 5. Develops, monitors and evaluates the effectiveness ofthe joint replacement program care pathway and modifies as necessary. 6. Manages chart and application reviews for qualityimprovement; analyzes and verifies that data collected by staff is complete,accurate, and consistent. 7. Analyzes the program data with the American JointReplacement Registry (AJRR) and uses it for quality improvement purposes. 8. Oversees patient education and preparation, resourcecase management, discharge planning and ensuring patient continuum of careservices. 9. Identifies cost-effective measures, makesrecommendations for the appropriate utilization of resources and promotespatient self-care management. 10. Works with the interdisciplinary teams to deliver anexceptional patient experience and monitors the quality-of-care delivery. 11. Evaluates and identifies patients' needs incollaboration with the Orthopedic Patient Care Coordinator. 12. When necessary, interfaces with primary carephysicians, specialists, and various disciplines on the development of plan ofcare. Refers to and supports approved joint replacement protocols. 13. In conjunction with the physician, implementscare/treatment plan by coordinating the episode of care for joint replacementsurgery patients across multiple providers/disciplines, ensuring patientcontinuum of care services and monitoring outcomes. 14. Acts as a clinical liaison with outside care providersto include home health nursing, PT, rehab and SNFs, as well as third partyadministrators. 15. Acts as a patient advocate and educator to assure thatthe patient has the knowledge to care for his/her condition and patient iseducated and empowered to be responsible for participating in the plan of care.Develops individualized patient/family education plan focused onself-management; delivers patient/family education specific to jointreplacement program. 16. Participates in data collection and analysis ofclinical outcomes of care and customer satisfaction standards. 17. Performs other duties as assigned.
