Job Description: I. Position Summary: HSL is dedicated to enhancing the quality of life for seniors through our affordable housing services. We empower residents to live independently while offering tailored support aimed at their well-being. As a Wellness Program Coordinator, you will play a crucial role in assessing residents’ health and wellness, creating personalized wellness plans, and connecting them to necessary services. The Wellness Coordinator will take a lead role in engaging residents through wellness assessments and health education programs. You will closely coordinate with our Nurse Care Manager and ensure that all interventions are well-documented and communicated. Your responsibilities will involve fostering effective partnerships with emergency responders, local providers, and payers to enable residents to maintain their independence safely. II. Achieve program goals independently while maintaining team alignment. III. Position Responsibilities: Conduct thorough wellness assessments to identify residents' needs and goals; create comprehensive wellness plans addressing various health domains. Coordinate care with primary physicians, mental health services, and hospitals. Collaborate with housing staff to customize programming that aligns with resident needs. Educate staff to identify and report any changes in residents' conditions promptly. Ensure residents have organized files containing essential healthcare information, end-of-life planning, and emergency contacts. Support on-site services and follow up with at-risk residents to encourage engagement in health and wellness activities. Establish relationships with payers that serve the senior population. Implement systems for effective communication between housing staff and service providers regarding residents' conditions and transitions. Encourage self-care among residents through tailored coaching and programming that supports their personal goals. Make referrals to local service providers as necessary, such as Visiting Nurses Associations and Adult Day Health Programs. Lead or participate in care planning meetings at the housing site. Assess residents with dementia and provide appropriate referrals. Collaborate with the Nurse Care Manager to plan and implement Evidence-Based Programs. Coordinate discharge planning effectively with local hospitals and other service providers. Assist in residents' transitions to different care levels as necessary, including facilitating family meetings. Advocate for residents, serving as a liaison with their families. Participate in tracking metrics and creating reports on program outcomes. IV. Qualifications: Masters degree in social work or a related field with 1-2 years of experience in aging services case management is preferred; a Bachelor's degree with 3-5 years of relevant experience is required. You should possess a genuine compassion for working with seniors and exhibit strong organizational and interpersonal skills for handling multiple projects effectively. A proactive, results-driven attitude is essential. Excellent communication skills and the ability to document information clearly in AASC Online software are required. Proficient computer skills, particularly in Windows, Word, PowerPoint, and Excel, are necessary. Some travel may be required in the Boston metro area for meetings and site visits. Certification in Dementia Care is preferred. Fluency in Cape Verdean Creole or Portuguese is a plus. V. Physical Requirements: Ability to lift, push, and pull up to 25 pounds, as well as stand, walk, drive, and sit during scheduled work hours. Salary Range: $53,971.45 - $80,957.18
